To report a complaint, including any adverse events, on Olympus products:

• To report a complaint or adverse event, call 1-800-387-0437 and ask for Regulatory Affairs
  or e-mail OCI-Regulatory@Olympus.com.
• To request service for a device that can be sent in for repair,
  call 1-800-387-0437 and ask for Customer Service.
